Abstract

The Government of Ethiopia aims to reform the health insurance system as one of the strategies to achieve universal health coverage. The health insurance is expected to provide sustainable financing for health, by replacing user fee with prepayment. To realize this strategy, the government has set a target of reaching 80% of the eligible population in 80% of the woredas in the country by 2020. In order to establish an affordable, feasible and sustainable insurance model, the Ministry of Health, Ethiopian Health Insurance Agency (EHIA), and Regional Health Bureau in Tigray began testing key changes in three pilot woredas in Tigray region, with technical assistance from the Clinton Health Access Initiative (CHAI). The purpose of this study is to assess the existing community-based health insurance business process in pilot woredas and establish a baseline of time and cost allocated for the undertaking the respective processes. A set of performance indicators for the time dimension were used to measure the amount of time consumed to complete CBHI related activities. Under normal circumstance, a time consumed for undertaking any activity involves two elements, namely lead time and throughput time. Lead time is the time taken to handle an entire case. Most CBHI related activities start from the verification of the membership status, which is usually done at the card room, although some facilities recheck the membership status at the other units too. The average time spent for membership verification is around 0.47 minutes per beneficiary; however, in most of the facilities members have to wait for a long queue prior to the verification process, which is subject to different sociocultural factors. On average a CBHI member has to wait 14 minutes before the membership status is verified. Facilities in Atsbi Wonberta have the highest queuing time at around 30 minutes per claim, while facilities in Kilite Awlalo have the lowest at 3.4 minutes per claim. The significant proportion of the allocated practical capacity was unused; i.e. 40.64%, 38%, and 54.87% of the supplied capacity was found unused by Atsbi Wonberta, Hawzen, and Kilite Awlalo schemes respectively.

This meta data contain data of process time, process cost and process quality community-based health insurance In general, a total of 6 Kebeles across the three pilot Woredas  of these are Kilete-Awlalo, Atsbi-Wemberta, and Hawzen.  were included in the assessment. On the health facility side, again purposive sampling was used to choose one primary, one general hospital, and five health centers. In most of the cases, facilities with high patient flow were chosen for the business process assessment. The analyzed data was available in excel format.
