Abstract	This survey was conducted by Goal Ethiopia in the Legambo Woreda of the south Wello Zone of the regional state of Oromiya. In the technical sense, the survey was conducted on selected clusters of household children under the age of five. In that regard, the objectives of the survey were: To estimate the prevalence of acute malnutrition among children aged 6 to 59 months (65-110 cm height/length when age is not known); To estimate retrospective mortality rate (CMR andU5MR); To estimate the morbidity rate in the two weeks prior to the survey date; to estimate the vaccination coverage for Measles & BCG and also Vitamin A supplementation for children 9-59 months and 6-59 months respectively; to assess the food security situation of the surveyed population. The findings of this survey revealed the following major points. 1. The prevalence of acute malnutrition based on weight-for-height z-scores was 11.2 % of them were classified as global acute malnutrition (GAM), 9.8 % of them were moderate and 1.4 % of children were severe acute malnutrition (SAM). ). No children were identified with nutritional bilateral edema. The crude mortality rate and the under-five children mortality were 0.26 and 0.18 deaths per death/10,000/day. 2. About 60% of the children suffered from diarrhea, 16 .6% from cough, 16.6 % from fever, and 6.6% from other diseases. The percentage of children vaccinated against measles by card was 19% and mother confirmation and the card was 91% TB (BCG) by scar 49.5% and those who received vitamin A in the last 6 months were 82.3 % of the respondents respectively.
