Abstract

The study was an open cohort study conducted in Addis Ababa and Wonji factory workers from 1997 to 2001. The main objective of the study is to identify simple markers (clinical or laboratory markers not requiring sophisticated laboratories) in the decision of initiation of antiretroviral therapy in resource-poor settings. Among HIV-infected Ethiopian cohort participants, 155 participants were included and simple markers predicting short-term death were examined using time-dependent Cox proportional hazards models. Timing of hypothetical treatment was compared between guidelines using the simple markers (based on presence of at least one marker), guidelines recommended by the United States Department of Health and Human Services (based on CD4 and viral load), and guidelines for resource-limited settings recommended by the World Health Organization. The study conducted in Addis Ababa(Akaki ) and Wonji on factory workers and was a national representative study. Simple markers (body mass index, HIV-related conditions, anemia, and lymphocyte count <1500/ul) were evaluated and compared with sophisticated laboratory markers for anti-retroviral treatment. From February 1997 to August 2001, 35 deaths were recorded among 155 HIVpositivee participants. Simple independent predictors of death were low body mass index, HIV-related conditions, anemia, and lymphocyte count <1500/ul. In such time as was covered by the study, 135 (87%) of 155 cohort participants would have had the same management under both the simple markers and the DHHS guidelines.

The study entitled simple markers for initiation of antiretroviral therapy for HIV-infected Ethiopians is aimed to evaluate simple laboratory markers like Complete blood count, Anemia, and BMI as an indicator of anti retroviral treatment rather than sophisticated laboratory markers in resource-limited settings.  The study is conducted among factory workers in Akaki and Wonji and a total of 155 participants included. Laboratory investigations like CD4 and viral load testing were performed for participants to compare the simple markers for ART initiation. The study gives information on whether simple markers were predictors for short-term death or not without sophisticated laboratory markers in resource-limited settings.
